The Brookline Center’s comprehensive community mental health care model excels in helping individuals experiencing multiple mental health, medical, difficult daily living and/or socio-economic situations. We provide care that recognizes, understands, and responds to the totality of a client’s life, including the many factors that can contribute to or drive poor mental health.
The Brookline Center offers two specialized programs for care and case management. For medical/mental health care integration, Healthy Lives partners with local hospitals and community health centers to integrate care, helping adults avoid crises that lead to ER visits and hospitalizations. For independent living case management, our Community Assistance Network (CAN) coaches people with serious mental health challenges to function better in daily life.
