Community Rehabilitation and Support (CRS) Program for Adults

Community Rehabilitation and Support services are directed toward adults with serious emotional disorders who are living independently in the community. Services are provided at the Center, in people's homes, or in the community depending on the situation. Case managers work with adults to develop an individual service plan tailored to the person's needs. Services include:

  • Financial planning – creating and managing a budget, applying for financial assistance
  • Housing assistance, e.g. applying for public housing, assistance/referrals for moving, etc.
  • Referral to employment services, e.g. Mass. Rehab, etc.
  • Making and following through with medical/dental appointments.
  • Household organization.
  • Applying for medical insurance.
  • Maintaining hygiene; creating a schedule for completing activities related to daily living
  • Other activities related to learning/obtaining life skills.

Eligibility for CRS services is based on an application to the Department of Mental Health.
